Job Description:
Job Description Chloeta is seeking an Optometrist to provide
direct eye care services to patients in a rural community in Crow
Agency, MT. This medical facility provides ambulatory and urgent
care services through the following departments: dental, optometry,
pharmacy, laboratory, radiology, mental health, medical social
services, and public health nursing. Travel will be required to
Crow Agency, MT to be the main point of service with visits to the
other sites as needed in Lodge Grass, MT and Pryor, MT. Supervisory
Responsibilities: This position has no direct supervisory
responsibilities but does serve as a coach and mentor for other
positions in the department Job Classification: Permanent;
Full-Time Duty Station: Crow Agency, Montana Travel: Yes, travel is
required to provide services between 3 different facilities.
Duties/Responsibilities Diagnosis of visual and ocular problems
through external and internal physical examination of the eye and
adnexa Treat and manage vision impairment and ocular disease with
corrective lenses, therapeutic pharmaceuticals and other
appropriate means Use all forms of topical ophthalmic therapeutic
medicine and oral therapeutic medicine to manage pathology and
trauma Performs minor surgical procedures including, but not
limited to, anterior segment foreign body removal, corneal
debridement and suture removal Provide emergency eye care services
on an outpatient basis, including the diagnosis and treatment of
ophthalmic trauma and pathology consistence with hospital
privileges Accept all emergency patients, referrals from an
outpatient clinic, the emergency room or consult throughout the
hospital as requested by medical personnel. It is expected that the
eye care provider will stay in clinic to treat emergency patients
beyond normal hours of operation as needed Utilize laboratory and
radiology services to evaluate ophthalmic and related systemic
conditions Direct to optometric technicians to assist in the
clinical examinations as appropriate Provide appropriate
documentation on rendered patient care utilizing the electronic
health record (EHR) system. The Contractor is responsible for
utilizing the EHR system to obtain, dictate, electronically enter
and assure preparation of appropriate medical records for patients
seen to assure the accumulation and organization of all pertinent
clinical data needed to provide comprehensive medical care.
Clinical data reports shall be completed in a timely manner and
shall be maintained in compliance with Joint Commission (JC)
standards Coordinate systemic care of ocular disease patients with
other physicians in the hospital Examine inpatients as requested by
attending physicians Consult with the Crow Service Unit, Chief of
Optometry and/or private ophthalmologic consultants and medical
practitioners as needed for complex cases Refer and coordinate
contract health care for patients requiring medical, surgical
and/or diagnostic services outside of the CSU and assists in the
coordination of patient transport for urgent/emergent ocular
conditions Any required treatments shall be provided or referred
appropriately following evaluation of the patient’s ancillary test
results Provide consultation on difficult cases to practitioners in
the same other specialties. Provides teaching to medical
practitioners, residents and students on the most complex cases
Communicate with the Health Center’s Health System Administrator &
Chief Medical Officer when appropriate Conduct accident
investigations and provides the results of all site incidents This

Education and Experience Required: Completion of an accredited
Optometry Residency Program with a minimum of six months’
experience in the specialty Required: Board certified with the
National Board of Examiners in Optometry, and shall possess a
current, valid, unrestricted Optometry License in any state, the
District of Columbia, the Commonwealth of Puerto Rico, or a
Territory of the United States Required: Current Basic Life Support
(BLS) Physical Requirements Work environment involves possible
exposure to hazardous materials, such as chemical, blood and blood
borne pathogens. Must be able to lift, push and/or pull 50 pounds
This position requires extended periods of being on your feet and
prolonged work sitting at a desk operating a computer. At the time
of hire, must provide documentation of evidence of immunizations as
required by the facility's immunization policy. License
verification and certifications must be provided at the time of
application Documentation as evidence of immunization as required
by NAIHS immunization policy for the following: Hepatitis A – Two
dose series at zero and six months. Hepatitis B – three dose series
at zero, one, and six months or documented immunity. A signed
declination of Hepatitis B vaccination series will be accepted.
Measles, Mumps, Rubella (MMR) – two doses or titer documentation.
Varicella (chickenpox) – two doses, titer documentation or history
of varicella/shingles. Tetanus, diphtheria, pertussis (Tdap/Td) –
one dose of Tdap, then tetanus (Td) booster every 10 years
thereafter. Documentation of annual TB test – may include Mantoux
TB skin test (PPD). All registered nurses require the 2 step TST.
Acceptable lab testing: Quantiferon Gold or T-Spot test. For
positive TB test results, please submit a copy of recent chest
x-ray results. Meningococcal–two vaccines (MenACWY and MenB)
recommended for microbiologist routinely exposed to N.
