POSITION SUMMARY:

The Community Health Worker supports the resource needs of our community's most vulnerable patients. This community facing position helps patients navigate social systems and barriers to care through referrals and direct services such as care coordination, delivering curriculum, Social Determinants of Health screening and assistance with resources and benefits.

COMPENSATION: $18.31-$18.91/hr DOE

EMPLOYMENT TYPE: Full Time

AVAILABILITY: Monday to Friday, 8am-5pm

MINIMUM JOB QUALIFICATIONS:

Education/Certificates/Licenses:

Required:

  • High school diploma or GED
  • BLS Certificate upon hire
  • Valid driver's license and insurability
  • Community Health Worker Certificate within the first six months of employment
    Experience:

    Required:

    • Proficient computer skills to include internet-based applications
    • Ability to perform in a fast-paced environment while managing multiple priorities
    • Ability to be well organized and accountable for time and activities with remote supervision
    • Must have a passion for serving others while adhering to and demonstrating exceptional attention to boundaries and professionalism
    • Must be a member of the community served and/or demonstrate involvement in community served
      Preferred:

      + 2 years' experience working in a health care or community service setting

          ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

          Duties and Responsibilities

          • Assists clients in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the Community Health program and the impact it may have on their wellbeing. Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
          • Documents all client encounters and referrals made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes achieved by client in an effective manner.
          • Advises client on the proper use of emergency rooms and provides information for alternatives. Supports patients in effective management of their prescribed treatment programs for chronic health conditions and self-care. Assists patient in understanding care plans and instructions. Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
          • Assists clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.
          • Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed. Follows up with both clients and providers regarding health/social services plans.
          • Facilitates communication and coordinate services between providers and the clients/patients. Coordinates and monitors services, including comprehensive tracking of clients' compliance in relation to care plan objectives.
          • Performs targeted outreach to patients identified through risk stratification, service contracts, payers, or other means to schedule appointments for patients recommended by the care team, and to connect patients with services within the organization and community.
          • Travels extensively to client homes, community locations, various agencies, and other outreach destinations.
          • Participates on community consortiums.
          • Works with teams on shared resource planning and development.
          • Involvement in public outreach events, advocating for the needs and perspectives of individuals and communities.
          • Follows safe home visiting protocols and uses good judgement.
          • Ability to be self-directed in unsupervised environments.
