In our medical and dental clinics, we provide care for 20,000
patients each year, more than half of whom are uninsured and rely
on our sliding fee scale. In our medical and dental clinics, we
serve all patients, regardless of their ability to pay.Home Care
and Hospice Program Overview: -RiverStone Health Home Care Services
provides medical treatment for an illness or injury, to help
patients recover, regain independence, and become as
self-sufficient as possible. Home health care helps patients manage
chronic conditions like heart disease, diabetes, pain management or
chronic obstructive pulmonary disease (COPD) and provides personal
care services. Home Care Services may be requested by physicians,
the patient or family member, or a hospital discharge planner.
Through consultation with the patient's healthcare provider, Home
Care Services develop a plan of care to meet the patient's
individual healthcare needs. The team includes nurses, physical
therapists, occupational therapists, speech therapists, medical
social workers, and certified home health aides.The RiverStone
Health Hospice Services care for terminally ill patients who want
to receive comfort-oriented care rather than continue to seek a
cure. Hospice care emphasizes quality of life. Care and grief
support services are provided wherever home is. Most hospice
patients receive care in their home, or the home of a relative or
friend. Hospice care can also be provided in nursing homes or other
facilities. Working together with patients and families, our team
of hospice professionals and volunteers help relieve physical
symptoms and decrease psychological, social, emotional, or
spiritual distress of patients and their caregivers. Our 24 hour
on-call nursing services are available to meet urgent needs. Job
Summary: The Director of Clinical Operations is responsible for the
overall direction of home health and hospice clinical services. The
Clinical Director establishes, implements, and evaluates goals and
objectives for home health and hospice services that meet and
promote the standards of quality and contribute to the total
organization and philosophy. Additionally, they provide guidance
and counseling to coordinators and Clinical Supervisors to assist
them in continually improving all aspects of home health care
services, provided through organization personnel. Duties also
include the development and implementation of quality improvement
initiatives and compliance programs for Home Care and Hospice.
Essential Functions/Major Duties and Responsibilities A. Director
of Clinical Operations 70%
- Coordinates and oversees all direct and indirect patient
services provided by clinical staff
- Oversees Clinical Supervisors in managing teams, planning of
workload, and organization of work in a clinical setting.
- Provides help in assessment, planning, implementation and
evaluation of patient and family/caregiver care to all clinical
personnel as indicated
- Assists the Senior Director in the preparation and
administration of the annual review of Home Health and
Hospice.
- Interprets operational indicators to detect census changes and
increases or decreases in volume, which could impact staffing
levels, revenues or expenses
- Assists Clinical Supervisors to develop skills and techniques
in evaluating the performance of staff and ensure consistency in
delivering clear expectations
- Hires, evaluates, and terminates organization personnel
- Conducts clinical performance evaluations annually, or more
frequently if indicated
- Oversees the maintenance of patient clinical records,
statistics, reports and records for purposes of evaluation and
reporting of organization activities
- Assures proper maintenance of clinical records in compliance
with local, state and federal regulations
- Responsible for orientation of new organization personnel,
either directly or by delegating to another staff member
- Plans and implements in-service and continuing education
programs to meet education and training needs of organization
personnel
- Assists with the evaluation of organization performance via
performance improvement program, productivity, quarterly and annual
reviews
- Assists in the development of organization goals, strategies,
policies and procedures.
- Assures compliance with all local, state and federal laws
regarding licensure and CHAP Accreditation.
- Serves as a backup to the Senior Director.QAPI Functions 25%
- Provide direction in the development and implementation of
quality improvement initiatives and quality monitoring activities
to include QAPI activities in Hospice and Home Care.
- Assist with identifying areas for improvement, gather and work
with others to analyze organization-wide data and report findings
to the Program Director.
- Educate Hospice/Home Care staff about quality improvement
systems, such as the Plan-Do-Study-Act (PDSA) Model for
Improvement. Ensure that projects use methodology as taught.
- Work with improvement teams to set goals, determine measurable
outcome, help identify changes to test, and determine process
measures and help team with Plan-Do-Study-Act (PDSA) cycles until
desired outcome is achieved.
- Coordinate the maintenance of clinical and direct care staff
generated client records in a manner that complies with applicable
standards and regulations
- Post team improvement project results and report progress to
senior leaders and staff on a regular basis.
- Provide technical improvement education and coaching to improve
team skills; ensure that the team is documenting progress and
activities.
- Provide leadership for the maintenance of accreditation status
to include all regulatory compliance reviews, surveys, or
investigations. -
- Attend meetings, conferences, workshops, and training sessions
and review publications and audio-visual materials to become and
remain current on principles, practices, and new developments in
assigned work areas.
- Communicate and coordinate regularly as appropriate with others
to maximize the effectiveness and efficiency of interagency
operations and activities.
- Assist and provide back up for Home Care and Hospice
supervisors with review on Plan of Treatment, orders, and plan of

Qualifications:
- Any combination of experience and training which provide the
equivalent scope of knowledge, skills, and abilities necessary to
perform the workPreferred Qualifications:
- Graduate from an accredited school with a nursing degree
- Minimum two years nursing experience
- Two years supervisory experience
- Experience working with underserved population
- Experience working in a customer service environmentRequired
Certificates, Licenses, Registrations:
- Montana State licensure as a Registered Nurse
- Current CPR certification
- Valid Montana driver's license
- Proof of vehicle insurance

- Some day travel is required to assess places where RiverStone
Health staff work, such as residences and health care
