Job Description:

Job Summary: Work in a multi-disciplinary approach to plan, coordinate, monitor, and supervise the provision of services to the consumers enrolled in the Serious Disabling Mental Illness (SDMI) Waiver Program. FLSA: NON-EXEMPT DUTIES AND RESPONSIBILITIES: Develops a written plan of care. In conjunction with the Mental Health Contract Social Worker and determine the services to be provided based on identified consume needs. Promote the consumer right to self determination. Assess consumer needs. Monitor service delivery. Maintain fiscal responsibility to the Department. Evaluate the provision of services for effectiveness, appropriateness, and quality. Assist consumers with determining the most efficient use of the SDMI Program. Identify and participate in the development of community resources to support consumers in their homes. Establish rapport with consumers, families/caregivers, agencies, medical professionals, Regional Program Officers, and other case management providers. Act as liaison for consumers, families/caregivers, and their health care professionals. Work under timelines as designated by the HCBS Team Supervisor and/or the Department for the most appropriate completion of job functions in relation to consumer and Departmental needs. Maintain consumer records as dictated by the Department and HCBS Team Supervisor. Miscellaneous duties as assigned Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations. Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict. Professional Requirements: Adheres to dress code. Completes annual educational requirements. Maintains regulatory requirements. Wears identification while on duty. Maintains confidentiality at all times. Attends department staff meetings as required within the department. Reports to work on time and as scheduled; completes work in designated time. Represents the organization in a positive and professional manner. Actively participates in performance improvement and continuous quality improvement (CQI) activities. Coordinates efforts in meeting regulatory compliance, federal, state and local regulations and standards Communicates and complies with the Benefis Health System Mission, Vision and Values as well as the focus statement of the department. Complies with Benefis Health System Organization Policies and Procedures. Complies with Health and Safety Standards and Guidelines. Education/Experience Requirements: Graduate of an accredited school of nursing, Bachelor in Nursing preferred or LPN, if state approved Current state licensure as a Registered Nurse or LPN if state approved. Certification in Case Management preferred. Experience as an RN, or LPN if state approved, commensurate with minimum of three (3) years of professional nursing experience. Knowledge of case management methods, practice, and procedure. Knowledge of the application of diagnostic and crisis intervention skills. Knowledge of issues and needs of long term care consumers. Prior knowledge of managing a budget. Prior experience with Medicare and Medicaid reimbursement. Knowledge of human behavior, disabilities, and the aging process. Knowledge of community resources.
